Denny & Associates, Inc. offers a customized consulting service for our individual clients. We want to make sure that you have a plan that best suits your needs and you understand the dynamics of your chosen program. In order for us to assist everyone during the open enrollment period, we ask you to follow the 4 EASY STEPS below.
STEP 1

We have two consultation options available. Please review the differences in services and cost for each option.
STEP 3
In order to customize your plan to meet your needs, we will need to research plan designs, costs, provider networks, and covered medications. This takes time and will need to be completed before our consultation. You may see a preview of the information that will be needed by clicking on the questionnaire preview link, but please click on the “Submit my Consultation” button that will take you to our secure site to submit the Questionnaire and Payment information.
STEP 4
Before your scheduled appointment, please review the CMS Model Consent Form. This form must be sign by each enrolling client. If you would like to see if you qualify for Advanced Premium Tax Credits to help you pay a portion of your premium, please also download and bring the Marketplace Tax Credit Questionnaire and the 2020 Income Estimate Worksheet. Again, these two documents are only needed if you are trying to apply for the subsidy. The Federal Poverty Guidelines Chart will assist you in seeing if your income qualifies for the assistance. If unsure, please complete the form and bring it with you.
